ABSTRACT

Objective:To observe the application of case-based problem-based learning (PBL) teaching in the postgraduate education of radiography. Methods:A total of 48 postgraduates in the Batch 2019/2020 of professional masters majoring in radiography were selected as research objects, and they were divided into observation group ( n=24, Batch 2020) and control group ( n=24, Batch 2019). The students in the control group were taught by traditional teaching, and the students in the observation group were taught by case-based PBL teaching method. The differences in imaging diagnosis level and structured assessment scores of the two groups of students and their evaluation of the teaching process were analyzed. SPSS 20.0 was applied to perform the t test and chi-square test. Results:The imaging diagnosis ability of the students in the observation group (84.98±7.51) was significantly higher than that in the control group (78.21±6.96) , and the difference was statistically significant ( P<0.05). The students in the observation group had higher ability of imaging diagnosis skills and case analysis and diagnosis during the structured assessment ( P<0.05). The students in the observation group had higher overall satisfaction with the teaching process than those in the control group ( P<0.05). Conclusion:The case-based PBL teaching is of great help in improving the clinical thinking and imaging diagnosis ability of postgraduates majoring in radiography. In the application, it is necessary to continuously improve and supplement the teaching implementation plan according to the teaching evaluation and feedback.

ABSTRACT

Alzheimer's disease(AD)and Parkinson's disease(PD)are common neurodegenerative diseases that seriously threaten the health of the elderly, involving various abnormalities in physiology and metabolism including the impairment of mitochondrial function, Ca 2+ homeostasis deregulation, oxidative stress, aggregation of misfolded proteins, autophagy and inflammation. However, mechanisms underlying the pathogenesis of these diseases have not been clearly elucidated, thus impeding advances in their treatment. In recent years, it has been found that mitochondria-associated endoplasmic reticulum membranes(MAM)play an important role in the development of AD and PD and exert their effects by regulating the functions of mitochondria and endoplasmic reticula. This article reviews studies of the past decade related to the effects of MAM on AD and PD, aiming to generate insights for exploring the constituent proteins of MAM and the molecular mechanisms of AD and PD via endoplasmic reticulum-mitochondria Ca 2+ transport and ER stress regulated by MAM.

ABSTRACT

BACKGROUND@#Andersson lesions (ALs) are not uncommon in ankylosing spondylitis (AS). Plain radiography (PR) is widely used for the diagnosis of ALs. However, in our practice, there were some ALs in AS patients that could not be detected on plain radiographs. This study aimed to propose the concept of occult ALs and evaluate the prevalence and radiographic characteristics of the occult ALs in AS patients.@*METHODS@#A total of 496 consecutive AS patients were admitted in the Affiliated Drum Tower Hospital, Medical School of Nanjing University between April 2003 and November 2019 and they were retrospectively reviewed. The AS patients with ALs who met the following criteria were included for the investigation of occult ALs: (1) with pre-operative plain radiographs of the whole-spine and (2) availability of pre-operative computed tomography (CT) and/or magnetic resonance imaging (MRI) of the whole-spine. The occult ALs were defined as the ALs which were undetectable on plain radiographs but could be detected by CT and/or MRI. The extensive ALs involved the whole discovertebral junction or manifested as destructive lesions throughout the vertebral body. Independent-samples t test was used to compare the age between the patients with only occult ALs and those with only detectable ALs. Chi-square or Fisher exact test was applied to compare the types, distribution, and radiographic characteristics between detectable and occult ALs as appropriate.@*RESULTS@#Ninety-two AS patients with a mean age of 44.4 ± 10.1 years were included for the investigation of occult ALs. Twenty-three patients had occult ALs and the incidence was 25% (23/92). Fifteen extensive ALs were occult, and the proportion of extensive ALs was significantly higher in detectable ALs (97% vs. 44%, χ2 = 43.66, P < 0.001). As assessed by PR, the proportions of osteolytic destruction with reactive sclerosis (0 vs. 100%, χ2 = 111.00, P < 0.001), angular kyphosis of the affected discovertebral units or vertebral body (0 vs. 22%, χ2 = 8.86, P = 0.003), formation of an osseous bridge at the intervertebral space adjacent to ALs caused by the ossification of the anterior longitudinal ligament (38% vs. 86%, χ2 = 25.91, P < 0.001), and an abnormal height of the affected intervertebral space were all significantly lower in occult ALs (9% vs. 84%, χ2 = 60.41, P < 0.001).@*CONCLUSIONS@#Occult ALs presented with more subtle radiographic changes. Occult ALs should not be neglected, especially in the case of extensive occult ALs, because the stability of the spine might be severely impaired by these lesions.

ABSTRACT

The development of nonalcoholic fatty liver disease (NAFLD) is closely related to the fatty acid (FA) uptake. This study aimed to investigate the effect of Krüppel-like factor 9 (KLF9) on CD36 (typical fatty acid translocase), hepatocellular lipid metabolism as well as the development and progression of nonalcoholic fatty liver. High-fat diet-induced obese C57BL/6J mice and db/db mice were used to test the expression levels of Klf9 and Cd36 in the livers. The primary hepatocytes were isolated from C57BL/6J mice, treated with Ad-GFP, Ad-Klf9, Ad-shCtrl or Ad-shKlf9, and then incubated with oleic acid and palmitic acid for 24 h. Liver-specific knockout of Klf9 mice were established. The protein levels and relative mRNA levels were examined by Western blot and real-time PCR, respectively. Triglyceride content was determined by using an assay kit. Lipid content was determined by Oil Red O staining. The results showed that: (1) Klf9 expression levels were increased in the livers of high-fat diet-induced obese mice and db/db mice, compared to their respective control mice. (2) Adenovirus-mediated overexpression of Klf9 in primary hepatocytes increased Cd36 expression and cellular triglyceride contents. (3) In contrast, adenovirus-mediated knockdown of Klf9 expression in primary hepatocytes by Ad-shKlf9 decreased Cd36 expression and cellular triglyceride contents. (4) Finally, Klf9 deficiency decreased liver Cd36 expression and alleviated fatty liver phenotype of high-fat diet-induced obese mice. These results suggest that KLF9 can regulate hepatic lipid metabolism and development of NAFLD by promoting the expression of CD36.

ABSTRACT

Rising emissions of greenhouse gases in the atmosphere have warmed the planet substantially and are also accompanied by poor air quality. The increased prevalence of allergic airway disease worldwide can be partially attributed to those global environmental changes. Climate change and air pollution pose adverse impacts on respiratory allergies, and that the mechanisms are complex and interactive. Adverse weather conditions, such as extreme temperatures, can act directly on the respiratory tract to induce allergic respiratory illnesses. Thunderstorms and floods can alter the production and distribution of aeroallergens while wildfires and dust storms increase air pollution, and therefore indirectly enhance health risks. Concentrations of particulate matter and ozone in the air have been projected to increase with climate warming and air stagnation, and the rising temperatures and CO2 increase pollen, molds, and spores, which escalate the risk of allergic respiratory diseases. The synergistic effects of extreme heat and aeroallergens intensify the toxic effect of air pollutants, which in turn augment the allergenicity of aeroallergens. With the Earth's climate change, migration of humans and plants shift the living environments and allergens of susceptible people. Urban residents are exposed to multiple factors while children are sensitive to environmental exposure. Since climate change may pose many unexpected and persistent effects on allergic respiratory diseases, health professionals should advocate for effective mitigation and adaptation strategies to minimize its respiratory health effects.

ABSTRACT

Objective To investigate the correlation between the expression of phospholipase Cβ1 (PLCβ1) and the clinical relevant parameters and prognosis in patients with hepatocellular carcinoma (HCC).Methods By using tissue microarray technique and immunohistochemical method, the expressions of PLCβ1 in tumor and pericancerous tissues were tested in 141 HCC patients. The relationship between the expressions of PLCβ1 and the clinical and pathological characteristics was analyzed.Colony formation assay and apoptosis experiments were used to check the effect of PLCβ1 on proliferation of HCC cells.KaplanMeier analysis and Cox multivariate regression model analysis were adopted to analyze the prognosis of HCC patients. Results The expression level of PLCβ1 in tumor tissues was obviously higher than that in pericancerous tissues, which was closely related to the tumor staging.Kaplan-Meier survival analysis indicated that the survival rate in HCC patients with high expression level of PLCβ1 was lower than that in HCC patients with low expression level of PLCβ1.Cox multivariate regression analysis revealed that high expression of PLCβ1 was an independent prognostic factor for HCC patients.Over expression of PLCβ1 in HCC cells could promote the proliferation of HCC cell and inhibit its apoptosis.Further investigation showed that activation of extracellular regulated protein kinase signaling pathway might be involved in PLCβ1-mediated HCC cell growth. Conclusion PLCβ1 can promote the progression of HCC,and the expression level of PLCβ1 can be regarded as an independent prognostic factor for HCC,and it is expected that PLCβ1 may become an ideal therapeutic target.

ABSTRACT

[Objective]To determine whether activated hepatic stellate cell(HSC)become quiescent phenotype if cultured in a soft extracellular matrix(ECM).[Methods]HSC-T6 cells which stably expressed myofibroblast(MFB) phenotype, were cultured in the 2D and 3D in-vitro matrix culture models that were constructed by 1 mg/mL fibrin gel. The proliferation activity of HSC-T6 was determined by cell counting kit-8(CCK8)assay at different time points(24 h, 48 h,72 h,96 h). Immunofluorescence and gelatin zymography analysis were performed to detect the expression of α-SMA,MMP-2,and MMP-9. Moreover, the cellular surface morphology and deformability was detected by AFM.[Results]After cultured in the fibrin gel,the cellular proliferation activity, expression of α-SMA,MMP-2 and MMP-9,and cellular deformability of activated HSC-T6 was obviously down-regulated in contrast to the control group (P<0.05),and there were also differences in the inhibition of cellular proliferation activity, the assembly of α-SMA stress fiber and the effect of cellular deformability between 2D and 3D fibrin gel cultured(P<0.05).[Conclusions]Soft fibrin gel inhibited the transdifferentiation of activated HSC-T6,but could not make it revert to α-SMA negative quies-cent phenotype.

ABSTRACT

Mucinous tubular and spindle cell carcinoma (MTSCC) is a rare sub-type of renal cell carcinoma (RCC). It has been considered to be a kind of "indolent" tumor with low-grade fashion, weak invasive capacity and relatively favorable prognosis. However, in the current case, a 3.7 cm×2.8 cm spherical mass with contrast enhancement was found in the left kidney incidentally by computed tomography (CT) in a 60-year-old male patient. A lesion in the right humerus (2.1 cm×1.6 cm×3.1 cm) was found at the same time without any symptoms or sign of pathological fracture by magnetic resonance (MR) imaging. Further positron emission tomography (PET)/CT scan which was ordered immediately after admission suggested multiple bone destruction including skull, pelvis, sternum, right humerus and femur, left scapula, multiple vertebrae and libs. Pathological examination after radical nephrectomy and palliative resection with internal fixation of the lesion in the right humerus indicated that both renal (3.0 cm×3.0 cm×2.5 cm) and bone lesions were MTSCC with the features of high-grade ovoid epithelioid cells, cord-like spindle cells and mucinous matrix under light microscope. The diagnosis of renal MTSCC concurrent with multiple bone metastasis was made. This case report suggested the necessity of general evaluation, especially bone scan for possible distant metastasis, as MTSCC might present unexpected advanced behaviors without any orthopedic symptoms. The behavior of bone metastasis might be associated with male and elderly age. MTSCC has similar enhancement features to papillary RCC on CT scan. As results, attentions are needed to differentiate MTSCC from papillary RCC as they both tend to show lesser enhancement degrees than cortex. Rather than exhibiting a dedifferentiating appearance, the pathological characteristics of bone metastasis lesion were close to those of primary renal lesion. The reason of distant metastasis to the bone remained unclear, negative expression of cytokeratin (CK) 7 might be attributed to. Though immunotherapy, chemotherapy and target therapy could all be methods for systematic therapies, procedures to remove renal lesions and prevent skeletal related events are still highly recommended.

ABSTRACT

Objective To investigate the effect of diabetes on clinical efficacy of transcatheter arterial chemoembolization (TACE) in the treatment of non-viral hepatitis hepatocellular carcinoma (HCC). Methods Retrospectively analyzed the clinical data of 367 non-hepatitis virus HCC patients treated by TACE, included 153 diabetes mellitus cases (test group) and blood glucose of 214 patients was normal (control group). To assess the treatment effect after 1 month of TACE based on response evaluation criteria in solid tumors, include complete response (CR), partial response (PR), stable disease (SD), progressive disease (PD), and calculate the disease control rate. Through 6 to 75 months follow-up to observed long-term efficacy, record the time to progression (TTP) and overall survival (OS) time. Survival rate were analyzed using Kaplan-Meier method and Log-rank analysis by SPSS 16.0. The single-factor analysis was used to analyze variables which variables that differed were analyzed by Cox regression. Results The disease control rate of test group was 69.9%(107/153) and control group was 74.3%(159/214), the difference was no statistically significant (P=0.125). The median time to progression (mTTP) and median overall survival (mOS) of test group were 10.0 and 15.0 months;and the mTTP and mOS of control group were 14.0 and 19.0 months, the difference were statistically significant (P=0.023 and P= 0.026). Tumor diameter ≥4.5 cm, numbers of tumor ≥3, invasion of blood vessels, α-fetoprotein≥200 μg/L, Eastern Cooperative Oncology Group score and diabetes were risk factors for OS of HCC patients. Conclusion Diabetes is unfavorable factors for overall survival of non-hepatitis HCC tread by TACE.

ABSTRACT

Objective To evaluate the effect ofcytochrome P450 isoenzyme subfamily 2C19 (CYP2C19)gene polymorphism on the clopidogrel antiplatelet therapy in cirrhosis patients after receiving transjugular intrahepatic portosystemic shunt (TIPS).Methods The clinical data of 171 cirrhosis patients, who were treated with TIPS during the period from January 2013 to December 2014,were retrospectively analyzed.During operation both the portal vein and the elbow vein blood samples were collected and sent for CYP2C19 gene testing. After TIPS,clinical follow-up checkup was made once every 3 months. The gene detection results and clinical follow-up findings were comparatively analyzed. Results A total of 110 patients, who had not received blood transfusion before TIPS and who had regularly taken clopidogrel antiplatelet therapy after TIPS were enrolled in the study. The mean time to take clopidogrel was 192.4 days (31-517 days),and the gene detection results of portal vein and elbow vein were quite consistent.CYP2C19 genotype of *1/*1 was found in 49 patients (44.5%),CYP2C19 genotype of *1/*2 in 27 patients (24.6%),CYP2C19 genotype of *1/*3 in 18patients (16.4%),CYP2C19 genotype of *2/*2 in 11 patients (10.0%),CYP2C19 genotype of *2/*3 in 3patients (2.7%),and CYP2C 19 genotype of *3/*3 in 2 patients (1.8%).Following-up examinations showed that the incidence of shunt dysfunction in patients carrying slow metabolic gene was 87.5% (14/16),which was significantly higher than that in patients carrying moderate metabolic gene (20.0%,9/45;x2=22.9,P=0.006)as well as in patients carrying fast metabolic gene (8.2%,4/49;x2=37.91,P=O.O00 1).Multivariate analysis of Cox regression model indicated that CYP2C19 slow metabolic gene variation was an important predictive factor for shunt dysfunction (95%CI:1.80-9.03,P=O.O00 7).Conclusion CYP2C19 slow metabolic gene variation, including genotype of *2/*2,*2/*3 and *3/*3,is an important factor that can influence the efficacy of clopidogrel treatment after TIPS.Preoperative CYP2C19 gene detection results can provide useful information, which is very helpful in making an effective and reliable anti-platelet treatment plan for patients after TIPS.

ABSTRACT

Objective To discuss the prognostic value of aspartate aminotransferase (AST) to neutrophils ratio index (ANRI) in patients with hepatocellular carcinoma (HCC) after receiving transarterial chemoembolization (TACE).Methods The clinical data of 107 HCC patients, who were admitted to authors' hospital to receive treatment during the period from January 2008 to June 2011,were retrospectively analyzed.TACE was successfully performed in all patients.Based on the 5-year overall survival rate,ROC curve was drawn and the cutoff value was determined.Preoperative ANRI,AST-lymphocytes ratio index (ALRI),AST-platelet count ratio index (APRI),neutrophil-lymphocytes ratio index (NLR),platelet count-lymphocytes ratio index (PLR) and other clinical pathological parameters were calculated.Univariate analysis, multivariate Logisitc regression analysis and Kaplan-Meier survival analysis were used to assess the value of the above indexes in prejudging the disease-free survival (DFS) and overall survival (OS).Results ANRI bore a close relationship to the presence of HBsAg,AST,presence of cirrhosis, tumor size, portal vein tumor thrombus (PVTT) and recurrence of tumor (P<0.05).Univariate analysis showed that ANRI,ALRI,APRI,NLR and PLR were significantly correlated with DFS and OS in HCC patients after receiving TACE (P<0.05).Logisitc regression analysis revealed that ANRI was independent factor influencing DFS and OS (P<0.05).Kaplan-Meier survival analysis indicated that the prognosis after TACE was poor in patients whose preoperative ANRI >7.8.Conclusion Preoperative ANRI level is an independent and effective predictor for judging the prognosis of HCC patients.A high preoperative ANRI level usually suggests a poor prognosis after TACE.
